| The Cyrus M. Quigley Foundation will seek to create opportunities for individuals to develop these qualities or foster them in others. The Foundation will focus on experiences that emphasize physical challenge and service to others. The Foundation can only consider grant proposals submitted by tax-exempt, not-for-profit [501(c)(3)] institutions. Requests must be submitted by March 1 of the the grant year. Grantseekers will be notified of the Foundation's decision by June 1 of the grant year. |

Together with reach!, our employee volunteer program, Quest Diagnostics has helped charities through fundraising efforts and by donating our time. The Quest Diagnostics Matching Gifts Program is designed to support those organizations whose mission is aligned with the companys Vision: Dedicated people improving the health of patients through unsurpassed diagnostic insights. Organizations must be recognized by the IRS as tax-exempt public charities and be based in the United States. Accredited educational institutions including public or private elementary, middle or high schools, junior colleges, colleges, universities or professional schools. Non-profit programs whose mission is to provide curriculum enrichment services to local schools are also eligible. |

QuikTrip Corporate Contributions - Multiple States

| QuikTrip receives numerous requests for contributions from very deserving agencies. In order to facilitate these requests and focus donations to selected causes, QuiKTrip has established guidelines and procedures. The first and largest recipient of QuikTrip donations is local United Way organizations. The company matches dollar-for-dollar all employee contributions. In addition QuikTrip provides significant funding to the Safe Place agencies that operate in its market areas. QuikTrip also encourages and supports Big Brothers Big Sisters agencies in all the markets through volunteering and financial grants. The remainder of the contribution budget is available to eligible 501 ©(3) nonprofit organizations, based in the communities where QuikTrip operates; Tulsa, OK, Kansas City, MO, Kansas City, KS, Wichita, KS, Des Moines, IA, Omaha, NE, Dallas-Fort Worth, TX, St. Louis, MO, Atlanta, GA., Phoenix, AZ. |
